Parental Responsibility Consent for Childhood Immunisations
Before any immunisation can be given to your child we have to obtain your informed consent, as the person who has parental responsibility.
If you are not the person who brings your child to their immunisations / vaccinations appointment, you must provide written and signed consent (a completed form) and ensure this is brought to the appointment. This form is available on the link below.

Information on travel vaccinations is available from the NHS website here
Vaccinations should be dealt with at least 8 weeks before your intended travel date. A Travel Risk Assessment Form must be completed for every traveller, and this form is then returned to the surgery. When the completed form is returned, an appointment will be made with the Practice Nurse, at least a week later, for the vaccinations to be administered. (A week is needed for the form to be checked and the vaccination(s) authorised.)
Only NHS routine vaccinations can be administered at the practice. Other vaccinations e.g. Hepatitis B, Rabies, Japanese B Encephilitis etc. may be recommended and you will be advised of this during your appointment. You will be advised to contact a private travel clinic, there are clinics available in Mansfield & Nottingham which provide the service.
