Parental Responsibility Consent for Childhood Immunisations
Before any immunisation can be given to your child we have to obtain your informed consent, as the person who has parental responsibility.
If you are not the person who brings your child to their immunisations / vaccinations appointment, you must provide written and signed consent (a completed form) and ensure this is brought to the appointment. This form is available on the link below.
