We now offer a choice of hospitals and consultants when you are referred by your GP to secondary care. For urgent appointments and some specialties we can make your appointment, whilst you are still at the Surgery, after your consultation. If you need more time to think about it then you can take some literature and a copy of your unique booking reference number home with you and you can contact the Choose and Book office whose number will be on the leaflet. There is also an option to book on-line following the guidance on the leaflet. All other referrals will be sent direct to the hospital and they will contact you with appointment details.
All the doctors are trained in family planning and will discuss this with you during normal surgery hours.
An additional family planning clinic takes place on Wednesdays between 12h00 – 16h00. Please note this is not a drop-in service, you will need to phone 01623 672260 to make an appointment.
All of the doctors undertake child health care surveillance clinics to which children under five years are invited at specific times.
These clinics are to monitor a child’s growth and progress and are useful in the early detection of problems.
Clinics run by the health visitors are on alternate Tuesdays between 1.30 – 3.00pm at Swan Lane Children’s Centre. Telephone 01623 666370.
These clinics are run by the practice nurses. Please make an appointment at reception. Asthma & Respiratory appointments are carried out as a telephone consultation.

All diabetic patients are encouraged to attend our clinic regularly for a check by the doctor and practice nurse. Please book your appointments via the receptionist. At the moment all reviews are carried out as a telephone consultation you only need to attend for the pre-check.

You may be required to provide a report, completed by your GP. Examples include: Medical reports for insurance companies, Fitness to travel forms. You might also need to gain Access to your GP records or request a Travel Vaccination.
We have a ‘Prices List’, itemising most of the reports and services.
When requesting a report, please note that time needs to be allocated to a GP (outside of seeing patients) for them to be able to research the information requested and to complete the requested forms and supply the requested information. Please discuss a time scale with our reception staff when handing in your request and allow for a minimum of two weeks.
For travel vaccinations you will need to allow for sufficient time, before your intended date of departure. You will be required to book two Nurse appointments. Please speak to one our receptionists for full information so that you can receive the vaccinations you need in good time, to enjoy your holiday.

Healthy Family Team – (Previously known as Community Health Visitors and School Nurses)
