You can now register with a Oakwood surgery using an online form.

The purpose of having a digital form is to make the process easier and more efficient for both patients and the practice. It has been proven to reduce the processing time of a registration form by up to 15 minutes and has improved the quality of the data provided.

You will be guided through the  online registration form and asked to complete similar questions to what is already included in the paper version. As a part of the process you will be asked to login to your NHS account if they already have one in order to verify your identity. When completing the demographic section of the form you will have the option to search for your NHS Number if you do not know it already. 

Once the registration has been completed it will be submitted and you will be provided with a unique reference number.
