DO I.... CHANGE MY PERSONAL DETAILS? If you move house, and/or acquire a new telephone number, please inform the receptionist of the relevant changes as soon as possible. If you move outside of the practice area, you may be advised to register with another doctor, as house calls may not be available. HOW DO I.... If you move out of your doctor's area or wish to change doctor for any reason, your medical card should be taken to your new doctor. If you are having problems, registering with another GP, please ring the Patient Service Department of the Nottinghamshire Health Authority - 0115 883 1820. |
