Well Person And Health Promotion Services

Diabetic Clinic

All diabetic patients are encouraged to attend our clinic regularly for a check by the doctor and practice nurse. Please book an appointment via the receptionist.

Asthma Clinic, Respiratory Clinic and Cardiovascular Clinic

These clinics are run by the practice nurses. Please make an appointment at reception.

Cervical Cytology

We recommend that cervical smears are carried out every three or five years, depending on age, and we will automatically invite ladies to attend our clinic when their smear is due. Any lady wishing to have a smear may make an appointment with the nurse, or their doctor if they prefer. When you phone please indicate if you are requesting a cervical smear test so that the necessary sterile instruments are readily available.

Minor Surgery

Several of the doctors are accredited to perform minor operations, inject joints, freeze warts etc. These are done at a special appointment after consultation with the doctor.

Other Services

To enquire about other services provided within the Health Centre building, contact the general office on 01623 420692. This office is open 8.30am - 5.00pm Monday to Thursday and 8.30am - 4.30pm on Friday.

Carers

Are you caring for someone registered with this practice? If you are it would be helpful if we had your contact details. However, you must have the person's consent before you do this. Carer registration and consent forms are available from reception.

Children

Vaccination And Immunisation

We advise that all children are kept up to date with their vaccinations - your health visitor and the surgery will keep you informed of the vaccinations your child needs. If you feel that your child is not up to date, please contact the surgery.

Children's Health

All of the doctors undertake child health care surveillance clinics to which children under five years are invited at specific times.

These clinics are to monitor a child’s growth and progress and are useful in the early detection of problems.

Clinics run by the health visitors are on alternate Tuesdays between 1.30 - 3.30pm at Swan Lane Children’s Centre. Telephone 01623 666370.

Young Persons' One Stop Health Shop

This is a drop-in clinic for those aged between 13 - 18 and is held on Fridays between 2.30 - 4.30pm. As well as attending to any health needs, C-Card registration and chlamydia screenings are available.

Insurance Certificates, Forms Etc

If you have a form which needs to be signed by the doctor, please leave it with the receptionist. There will be a charge to pay. For most forms there is no need to see the doctor to have them completed: the receptionist will advise you. Please note: due to the high volume of paperwork, you should allow several weeks for their completion.

HGV, PSV And Other Non-NHS Medicals

These require an appointment with the doctor outside surgery hours. Please arrange these with the receptionist in good time. A fee as recommended by the BMA may be payable for these examinations.

Family Planning Services

All the doctors are trained in family planning and will discuss this with you during normal surgery hours. An additional family planning clinic takes place on Wednesday between 12.30 - 2.00pm. Please phone 01623 848802 to make an appointment.

Referrals to Secondary Care (Hospital)

We now offer a choice of hospitals and consultants when you are referred by your GP to secondary care. For urgent appointments and some specialties we can make your appointment, whilst you are still at the Surgery, after your consultation. If you need more time to think about it then you can take some literature and a copy of your unique booking reference number home with you and you can contact the Choose and Book office whose number will be on the leaflet. There is also an option to book on-line following the guidance on the leaflet. All other referrals will be sent direct to the hospital and they will contact you with appointment details.
